Short answer

No controlled trial has shown that cannabis or cannabidiol repairs muscle, tendon, or bone. Trials measure pain scores, sleep, and self-reported function. Results on pain are mixed. THC remains banned in competition by the World Anti-Doping Agency. CBD was removed from the WADA Prohibited List in 2018.

What each governing body allows

  • WADA: THC and other cannabinoids sit in class S8, banned in-competition. The urine threshold for THCCOOH is 150 ng/mL.
  • NCAA: THC is banned. CBD was removed from the banned drug list in 2018.
  • US federal law: cannabis is Schedule I under the Controlled Substances Act. The FDA has approved one CBD drug, Epidiolex, for two rare forms of epilepsy. No cannabis product is FDA approved for recovery or athletic pain.

Evidence on pain and swelling

A 2017 review by the National Academies of Sciences, Engineering, and Medicine found moderate evidence that cannabis treats chronic pain in adults. Most of those trials studied people with nerve pain or cancer pain, not athletes. Sample sizes were small, often under 100 subjects. Ice, NSAIDs, load management, and physical therapy remain the standard for acute sports injury.

Delivery formats

Route changes onset and dose. Inhaled products act in 1 to 10 minutes. Oral capsules and edibles absorb slower, with peak blood levels at 1 to 3 hours. Oral CBD bioavailability in trials runs near 6 percent. Topicals cross the skin in small amounts, so blood levels stay low.

Convenience shapes product choice. Gummies and tinctures need no device and no smoke. That ease does not change how the drug moves through the body.

Before you use a product

  • Read the label for THC content.
  • Check the competition window. In-competition bans apply for days after use.
  • Ask for a third-party lab test. Testing rules vary by state.
  • Tell a clinician about other drugs. CBD blocks CYP enzymes and can raise blood levels of some prescriptions.

What is unknown

Dose, timing, and product type for recovery are undefined. No study has measured return-to-play time after cannabis use. Long-term effects on tendon healing in humans are unknown.
