Short answer
Cannabis creams are topical products that carry THC, CBD, or both. A user rubs the cream into a joint, a muscle, or a patch of skin. The goal is relief at that spot without a high. Clinical evidence for this use is limited. Reviews of topical cannabinoids report small trials, mixed results, and a high risk of bias. Convenience, not proof, drives much of the demand.

How a topical works
Skin holds cannabinoid receptors. THC binds CB1 and CB2 receptors in the epidermis, hair follicles, and sebaceous glands. CBD acts on several targets, including TRPV1, the same receptor that responds to heat and capsaicin.
Two product types behave in different ways.

- Creams, lotions, and salves. These sit on the skin surface. Little active compound reaches the bloodstream. Most users report no psychoactive effect.
- Transdermal patches. These add penetration enhancers. Some deliver THC into circulation. Patches can produce a high and impairment.
Onset for a cream runs 15 to 30 minutes. Effects last 2 to 4 hours. Users who feel nothing after 30 minutes often apply more. That raises cost, not blood levels.

What the evidence shows
Published reviews indexed in PubMed find few controlled trials of topical cannabinoids for pain. Sample sizes are small. Several studies lack a placebo arm. Results for arthritis, neuropathy, and muscle soreness do not agree.
The NIH National Center for Complementary and Integrative Health states that evidence on cannabinoids for pain is mixed, and that topical forms are understudied.
The FDA has approved no cannabis or CBD product for pain. The agency has sent warning letters to companies that market CBD creams with claims to treat disease.
Reading a label
Labels list milligrams of THC and CBD per container, not per dose. A 1 oz jar holds about 30 mL. A jar with 300 mg CBD works out to 10 mg per mL. A pea-sized dab of about 0.1 mL carries about 1 mg of CBD. That math matters when a label shows a large total number.
State-licensed products carry batch test results for potency, pesticides, mold, and heavy metals. Ask for the certificate of analysis.
Why convenience decides
- No inhalation, no device, no odor.
- Hemp-derived CBD creams with no more than 0.3% THC are sold in pharmacies, grocery stores, and online. THC creams sell at state dispensaries.
- THC topicals are illegal under federal law. Hemp-derived CBD is not.
- TSA permits hemp-derived CBD in carry-on and checked bags. THC products are not allowed in either.
What remains unknown
No standard dose exists. No agreed THC-to-CBD ratio exists. No list of conditions that respond exists. Relief reports are user reports, not trial data. A clinician can review interactions with other drugs.
